Max's Mission would like to raise awareness about suicide with the "Say Their Name" project.  If you want to participate please fill out this form, and submit a photo of your loved one along with answers to the questions.

We will share your loved one's photo along with the answers to the questions below on Max's Mission social media accounts on Instagram, FB and LinkedIn.

Thank you for your participation in helping to destigmatize suicide and put a face to the names of the over 700,000 people that die by suicide each year in the world.

Support National Suicide Prevention Awareness Month
